Specialty Exams

CNOS Orthopaedics and Neurosurgery departments provide independent medical exam’s (IME) and opinions that fall outside the normal scope of usual medical treatment. Physical & Occupational Therapy

Scans & Testing

The most advanced technology and caring staff members: it’s all at CNOS. We’ll use methods such as MRI tests and neurological EMG injections to get a close, comprehensive look at your condition. Then, we’ll recommend your best treatment option available.

Immigration Physicals at CNOS

Our experience Occupational Medicine team performs Immigration Physicals at our Dakota Dunes Clinic location (575 Sioux Point Road). These physicals are administered by Dr. Douglas Martin and Sarah Pluger, PA-C.

The following is required for your appointment: (PLEASE REVIEW)
  • * Non-English-speaking patients are required to bring their own interpreter to this appointment. Interpreter must be able to speak, read and write in both the patient’s native language and English.
    • CNOS will not provide an interpreter. Telephonic/video interpreter cannot be accepted.
  • Government issued photo ID for patients ages 14 and older (passport and/or driver’s license)
    • Patients under the age of 14 will need to provide a birth certificate or proper affidavits.
  • Vaccination records (past and current) – prior I-693 forms cannot be accepted.
    • lab and/or x-ray results for past positive TB test.
    • Proof of treatment for active or latent TB.
  • Proof of COVID-19 vaccination
  • I-693 Forms with parts 1, 2, 3, & 4 on pages 1-3 completed. (included/attached)
    • Name and A – number (if any) must be filled in at the top of pages 2-14.
  • *Parent and/or legal guardian must attend for all patients under the age of 18.


The fee for the examination is $275. There are often additional fees for required lab work and/or vaccinations.


Labs Offered:

  • VDRL
  • *T-Spot (tuberculosis)
  • Gonorrhea
  • Chest X-Ray (2V) (if positive TB test)

Immunizations Offered:

  • Influenza
  • Hepatitis A & B
  • MMR
  • Tdap (tetanus)
  • Varicella
  • COVID-19

Worker’s Compensation Services at CNOS

We’ve developed Workers’ Compensation Surgical Care and Medical programs that provide the services you need. No matter the injury, the road to recovery requires coordination among many different groups. Our dedicated workers’ compensation staff members serve as liaisons between patients, physicians, insurance companies, case managers and employers during the approval process.

Our experts provide help and healing for workplace injuries along all of our major service lines. Treatments come in various forms, as with any health condition, and will depend on the injury.

A team of CNOS Staff will:
