Medicare’s Physician Quality Reporting Initiative (PQRI)

Dear Patient:

Your doctor is taking part in Medicare’s Physician Quality Reporting Initiative (PQRI). This initiative will help your doctor and Medicare provide the highest level of quality of care for people like you, who have Medicare. Medicare greatly appreciates that your doctor has chosen to join this important initiative.

How Does PQRI Work?

You doctor is reporting information to us about specific services provided frequently to Medicare patients with certain medical conditions. This information will help your doctor and Medicare measure the quality of care.

What Does This Mean For You?

When you get any services related to the quality measures that your doctor is reporting to Medicare, the reported quality codes will show on your Medicare Summary Notice (MSN) with the following statement:

“This code is for informational/reporting purposes only. You should not be charged for this code. If there is a charge, you do not have to pay the amount”

Please be assured that your doctor’s reporting of quality measures for these services won’t result in any additional cost to you.

Where Can I Learn More?

For more detailed information about this initiative, visit www.cms.hhs.gov/PQRI on the web. Or, call 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048. Medicare thanks you and your doctor! The Physician Quality Reporting Initiative will help ensure that all Medicare patients get the quality of care that they deserve.
