How do I pay my bill?

How do I refer a patient?

We know you have a choice when it comes to referring your patients for specialized health care. The physicians and medical network want to thank you for placing your trust in CNOS.

When calling to schedule an appointment for your patients, please make sure to ask for the following in the patient’s priority order: 

  • Physician
  • Location
  • First available appointment

How can I receive a copy of my medical records?

Please print and fill out our Patient Authorization for Release of PHI form and fax to 605-217-2922.

Limited health information can also be found on your patient portal at www.followmyhealth.com.

Navigating your virtual visit appointment

1. Use a device with a camera/microphone

  • Use a computer with Chrome, Firefox, or Safari
  • OR use an Android or iPhone

2. Accept the invite

  • Click on the invitation link that was sent to your email or via text message
  • OR copy and paste the link into your browser

3. Check in

  • Enter your name and check in to your appointment

4. Grant access to your camera

  • Allow the application access to your camera and microphone

5. Your doctor will begin the session

  • Secure, HIPAA compliant, no software download, no registration needed

    Existing patients can ask questions through the patient portal. If you have never used the patient portal before, please send an email to cnos@cnos.net requesting access or call us at 605.217.2667 and we will send you a link to join the portal.

    New patients can call 605-217-2667 (opt. 3) to contact the nurse triage area with medical questions.

    What are your regular office hours?

    Our clinic hours vary based on location, so please see our locations page for specific hours by location.

    Am I qualified to use your Orthopaedic Urgent Care?

    ortho urgent care logoOur Orthopaedic Urgent Care Clinic treats orthopaedic conditions, so we accept patients with most bone and joint problems including:

    • Fractures, sprains and strains
    • Sports injuries
    • Dislocations
    • Conditions of the hip, knee, shoulder, elbow, hand, wrist, foot and ankle
    • Back and neck injuries

    Please note that we are not able to treat head injuries that may require emergency care like trauma from motor-vehicle accidents or concussions. Since this is an outpatient facility, we do not store emergency medical equipment like narcotic medications, IVs, or materials for complex sutures. If you come and need these services, we may send you by ambulance to the ER. If you’re not sure, you can call us at 605-217-2667.

    CNOS offers same-day access to emergency orthopaedic care for patients of all ages, and new and existing patients. If you have a question about an injury and you can’t wait, please call us at 605-217-2667 to speak to an Orthopaedic Specialist. *Minors must be accompanied by an adult*
