How can I pay my bill?

You can pay your bill online by accessing our patient portal at www.followmyhealth.com, or by calling 605-217-2667 (opt. 5).  If you have never used the patient portal before, please send an email to cnos@cnos.net requesting access or call us at 605.217.2667 and provide us with your email address and we will send you a link to join the portal.


 

How can I get a copy of my medical records?

Please print and fill out our Authorization to Release Information form found here and fax to 605-217-2922.  Limited health information can also be found on our patient portal at www.followmyhealth.com.


 

What is a mid-level provider?

Our physician assistants (or PA) and nurse practitioners are nationally certified and state-licensed medical professional that practices medicine under the supervision of a physician. Midlevels can take your medical history, conduct physical exams, diagnose and treat, order test, assist in surgery, and write prescriptions.


 

Who can I call with medical questions?

Existing patients can ask questions through our patient portal at www.followmyhealth.com.  If you have never used the patient portal before, please send an email to cnos@cnos.net requesting access or call us at 605.217.2667 and provide us with your email address and we will send you a link to join the portal.

New patients can call 605-217-2667 (opt. 3) to contact the nurse triage area with medical questions.


 

What are your regular office hours?

Our regular clinic hours are Monday-Friday, 8am-5pm.   The Morningside and Northside clinics close at noon on Fridays. Please see our locations page for rehab hours by location.

Northside and Morningside Clinics 
Monday – Thursday 7 am – 5pm,
Fridays 7am – 12pm.

Dakota Dunes Clinic
Monday – Friday 8am – 5pm


 

How do I get a refill on my prescription?

You can request a refill on prescriptions through our patient portal at www.followmyhealth.com or via your pharmacy.


 

At which Siouxland hospitals do CNOS physicians care for patients?

CNOS physicians are affiliated with Dunes Surgical Hospital, Mercy Medical Center, and Unity Point Health-St. Lukes.


 

Where can I be seen by a CNOS provider?

In addition to our CNOS clinic locations in Dakota Dunes, Sioux City, and Moville, CNOS providers see patients at various satellite locations in Iowa, South Dakota and Nebraska.  A full list can be found here.


 

Do I need a referral from my primary care physician to see a specialist?

Rheumatology and neurology require a referral.  Orthopedics and neurosurgery do not.
